*Multiple registration rate is per additional attendee. Discount available only to colleagues registering at the same time from the same company. First registrant pays full rate. Please copy this form and fill out registration information for each attendee.
PAYMENT
Payment must accompany registration. Registrations without payment will not be processed. Company checks must be made out to PennWell / Lasers & Photonics Marketplace Seminar 2009 and payable in U.S. dollars drawn on a U.S. bank. Credit card payment requires card number, signature and expiration date. All domestic and international payments must be received on or before Monday, January 26, 2009. No attendee will be admitted into the Seminar without payment. Cancellations made in writing to the registration department by January 7, 2009 will be accepted subject to a $195 cancellation service charge. Confirmed registrants who do not attend the show or cancel after January 7, 2009 will be liable for the entire registration fee. All cancellations must be received in writing. Non-payment does not constitute a cancellation. Substitutions may be made at anytime.
